Florida's Blood Centers, Inc., (FBC) in Orlando Florida is requesting permission to use my name, voice and or/likeness in FBC marketing program.

In consideration of my inclusion within the FBC marketing program, I grant Florida's Blood Centers, and anyone authorized by FBC permission to use my name, voice and/or likeness in the program and in any program, I waive any rights of privacy and/or publicity that I might otherwise have with regard to the program and any derivative work of the program, including any promotional materials in connection with the FBC marketing program in any media (now known or hereafter developed) throughout the world. I will not receive, and do not expect to receive compensation as a result of any marketing or advertising, or use of my name, voice and/or likeness as described herein. I understand that FBC has the exclusive right to use my name, voice and/or likeness in whatever way it wishes.

No use of my name, voice and/or likeness shall be the basis of any future claim of any kind against FBC and affiliated companies, the officers, directors or employees of any of these, or their successors or assigns, nor shall this release be made the basis of any such claim. I understand FBC may contact me for further information prior to publishing my submission.

FBC employees and their immediate family are not eligible to win. Must be a registered donor with FBC to be eligible to win. Odds of winning iPad dependent upon the number of entries received. Non-transferable. No cash value. Contest ends February 28, 2011. One entry per person. Winning entry will be drawn on Friday, March 4, 2011.
